I'd take a look at the tank from the bottom.  About half of those I have seen
have been "adjusted" by a floor jack.  An unknowing PO or mechanic looks
under the van, sees the flat surface of the tank, positions the jack, and
"crunch," it now holds three gallons less.  With mine, the fuel level sender
would show I still had some gas, but would run out.  I had to go by number of
miles on the odometer.  YMMV
