Remove the knob for the switch.  It pulls straight off.
Slide all the ventilation controls to the far left (drivers side).
The face plate for the ventilation controls can be carefully pried up on
the side next to the fan switch.
Remove the ashtray.

You should be able to reach in and wiggle it out now.  Might be a little
easier if you remove all the knobs and take the face plate all the way
off.
