Thank you all for your help on the pulleys.  It seems that the shop that installed my water pump in January, installed the pulley on it backwards, causing it to be
about 1/4 inch off line.  No one can explain how I've been able to go 3-4000 miles in the last 8 months without the belt coming off.

It is now fixed.
