I called up Tiico and got a replacement from them.

I believe there are two types, one with 6 bolts, and another with 4.
The first one they sent me was the wrong type (had 4 bolt holes, not
the 6 bolt hole type I needed.) I still have that one, if it is the
type you need (I'm up in Tyson's Corner, FWIW.)
