Excessive blow-by from bad rings-pistons will also do this.

I'd say it's one of two things (both of which I've experienced with
similar
symptoms) - either a problem with the injection pump mounting, causing the
pump to twist slightly and alter the timing, or a plugged air filter
causing early
stages of runaway. Both are serious issues, so check 'em both - ie: check
those inj. pump bolts, and check your air filter...
