I used to have a small CB mounted in my Westy. I mounted it between the visors on the headliner. I removed just the front screws on the headliner and that gave me enough room to mount it and run wires and antenna. I mounted the antenna in the center of the luggage rack which made it easy to run the antenna lead but it did require drilling a small hole in the metal roof under the luggage rack. I could screw in the antenna by standing on top of the wheel well. This is the highest place I could mount it on the van and it worked pretty well.
