What y'all are describing sounds a lot like my vacatino last year.
Once I got up into the mountains around Kalispell, MT, the fan finally
quit completely.  Is was only then that we found the culprit - the
ground wire to the ECU was loose.  Half a screwdriver turn and all was
well.
