So far as the accelerator pedal detaching from the floor, that is (or was when it happened on mine) simply a matter of the pedal releasing from the snap-on bracket that holds it.  Not a big deal, just fit it back on.  You may have pulled on it when looking at the clutch pedal?  Of course, I'm no expert, but mine did that one time a couple of years ago.  It has never happened since.  mcneely
