2.  After successfully instaling a Time Sert(tm) steel insert to replace
stripped threads on my oil drain plug,  the next oil draining the threaded
piece and drain plug came out together

This should have never happened. Did you use Time-Sert's kit? The bottom
threads will lock it into place without the use of loctite. Did you follow
the direstions on step 5 of their instructions??
